I applied for mortgage re-finance at Wells Fargo, holder of my current mortgage. The GFE contains several fees that I was not charged on my previous refinancing. e.g., "Florida Mortgage Tax", which they say is required by law. Is that true?

The other one that really annoys me is "Escrow Waiver Fee". This is a fee for not having the lender impound my taxes and insurances (I pay them myself directly). Is this legal? Do I have to agree to this?
